- The Consulate General of Brazil in Toronto issues death certificates for Brazilian citizens who have died in Canada , in the provinces under its jurisdiction : Ontario (except Ottawa ), Manitoba and Nunavut.
- To produce juridical effects in Brazil, the death certificate issued by the Consulate must be confirmed in Brazil through the "1 ° Ofício do Registro Civil" of the location the deceased had a legal domicile in Brazil, or in the "Cartório de 1° Ofício de Registro Civil", in Brasilia, Distrito Federal (Federal District) (Brazilian Law N° 6.015, from the 31 st of December, 1973).
2. REQUIREMENTS
- The applicant for the death certificate must preferably be a Brazilian citizen and come personally to the Consulate.
Documents
a) Official death certificate issued by the Canadian Registrar General " long form" ( * );
b) Cremation certificate, when applicable ( * );
c) Embalmment certificate, when applicable ( * );
d) Medical statement indicating the causa mortis ( * );
e) Original passport of the deceased (for cancellation); and
f) Valid identification document of the applicant ( * ).( * ) original and a photocopy.
- If the remains of the deceased have to be transported to Brazil , it is necessary to legalize at the Consulate all the original Canadian documents mentioned in item 1, except the Canadian death certificate and the valid identification of the applicant.
Form
- Fill up, date and sign the death certificate form.
Fee
- The issuance of a death certificate does note require any payment.
3. PROCESSING TIME
- The processing time for the issuance of death certificate is 05 working days when there is no immediate need to transfer the remains of the deceased to Brazil.
4. RE-ISSUANCE OF A DEATH CERTIFICATE
- The Consulate General of Brazil in Toronto only re-issues a death certificate for the Brazilian citizen whose death has been previously registered at this Consulate. For other cases, the interested party must apply for a re-issuance at the Brazilian Notary Public or Consular Office which issued the original death certificate.
Fee
- The consular fee for the re-issuance of a death certificate is Cnd $9,00, which must paid by money order or certified cheque to the " Consulate General of Brazil in Toronto " .
Processing time
- The processing time for the re-issuance of a death certificate is 05 working days.
5. MAIL SERVICE
- The documents can be submitted previously by mail to the Consulate. Nevertheless, the applicant must come personally to the Consulate, upon receiving the death certificate to sign the Registry Letter before the consular authorities.
- If the documents are sent by mail, do not send original documents, but authenticated copies which will be held by the Consulate.
- Please inform the date in which you will come to the Consulate to sign the Registry Letter, taking into account the five working days for the processing of the document.
6. ATTENTION
- Make sure to submit complete applications with all documents. Incomplete applications will be automatically be returned to the sender.
